Medical Condition of Players

• All registration Forms have medical details appearing on them
• Registration Forms held by age group Head Coaches
• Reviewed monthly
• Procedure already in place

Note to Parents/Carers
Parents/Carers must make sure that their coach is informed if their child has any medical problems or if there are any changes to said conditions.

Note to Head Coaches
You will be given details of members of your age group from the central database that has been prepared from completed registration forms. These will be updated to you on a regular basis.
